You need to stabilize the shank of the hook to stop it twisting and keep the point inline with the direction of removal. It is by far the safest and easiest removal technique. That's where the fun begins. I don't recommend pulling it through. I found sliding a knife blade down and using that to push the skin away from the barb worked best for me.
